You have the right to access and obtain copies of your medical records. Saltmarsh Island Health makes it easy to request your records securely.
How to Request Records
- Online: Request through your MyChart patient portal
- Phone: (555) 123-4570
- Fax: Complete authorization form and fax to (555) 123-4573
- In Person: Visit our Medical Records office at Harbor Hollow Hill Campus
What You'll Need
- Completed Authorization for Release of Information form
- Valid photo ID
- Specific date range or records requested
Processing Times & Fees
Most records requests are processed within 5-7 business days. There is no charge for the first 25 pages; additional pages are $0.25 per page. Electronic delivery via MyChart is free.
