Your Rights as a Patient
At Saltmarsh Island Health, you have the right to:
- Respectful Care: Receive considerate and respectful care regardless of age, race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sex, sexual orientation, or gender identity.
- Privacy and Confidentiality: Have your health information kept private and confidential in accordance with state and federal law.
- Information: Receive information about your diagnosis, treatment, and prognosis in terms you can understand.
- Participation in Care: Participate in decisions about your care, treatment, and services.
- Refuse Treatment: Refuse treatment to the extent permitted by law and to be informed of the medical consequences.
- Access to Records: Access your medical records and request amendments as permitted by law.
- Voice Concerns: Voice complaints about the care you receive without fear of reprisal.
Your Responsibilities as a Patient
To help us provide the best care, we ask that you:
- Provide complete and accurate information about your health
- Follow the treatment plan agreed upon with your healthcare provider
- Keep scheduled appointments or notify us if you cannot
- Ask questions if you do not understand your care
- Treat staff, other patients, and visitors with courtesy and respect
- Meet your financial obligations
